An exciting opportunity has arisen to join the Transitions Team. The team is currently subject of hybrid working arrangements with an expectation of being office based (location; Sparkbrook Centre 34 Grantham Road Sparkbrook Birmingham West Midlands B11 1LU) 2 days per week this being a non-negotiable requirement for this role. Successful candidates will work with caseloads and citizens as part of its citywide service.
The Transitions supports 17-25 year olds who has a diagnosis of Learning Disability Physical Disability and Autism as they transition to adulthood. We are a unique service working across Birmingham City Council and Birmingham Childrens Trust offering statutory support for young people leaving the educational or care system with care and support needs. This exciting opportunity has arisen for Social Work practitioners who have practiced at Grade 4 level and have a qualification in Social Work to join the Transitions Service. We are looking for an organised and dependable team players who have a positive and flexible approach with excellent communication and interpersonal skills along with being enthusiastic and self-motivated.
You will be expected to prove your ability to work independently and to at all times make defensible decision-making decisions and exact your professional curiosity when offering support to Transitions citizens; the collective delivery and embedding of the Transitions Service whole system priorities using strength based whole system approaches will be essential and you will be offered high level supervisory support at all times.

You must be able to apply the principles and process which accompany the 3-Conversation Model and the Care Act 2014 to deliver on person centred practices where citizens are enabled to exercise choice control independence and build resilience with a focus on wellbeing prevention and early intervention to support.
You must be able to demonstrate partnership working with other professionals both internally and externally. You must be able to demonstrate your excellent application of the roles and responsibility of a social worker. Within your role you will be expected to use innovation where appropriate to resolve challenges within your caseloads that you will be responsible for.
You must be able to demonstrate your knowledge and experience of the Continuing Health Care process / appeals and you will be skilled in Section 117 aftercare planning and reviews. You will demonstrate your ability and experience in working through disputes on a professional basis with other disciplines and you will support other team colleagues in disputes where required.
