Description

JOB TITLE: Day Supports / Center for Learning - DIRECT SERVICE PROVIDER (DSP)


I. Minimum Education/Experience

High School Graduate or GED Certificate required. Preference will be given to those applicants who have experience working with individuals with intellectual and developmental disabilities.


II. Essential Job Functions

  1. Ability to navigate on a desktop laptop or tablet.
  2. Ability to type.
  3. Meet the personal care needs of each resident including toileting skin care bathing feeding etc.
  4. Assist with nursing procedures and treatments as indicated or assigned.
  5. Report immediately all concerns to QP.
  6. Follows policies and procedures objectives quality assurance program safety environmental and infection control standards.
  7. Assist and accompany residents on field trips and other recreational activities.
  8. Provide and participate in routine and assigned housekeeping duties and keep the program areas neat clean and safe.
  9. Participate provide and assist with active treatment according to an individuals Individual Service Plan.
  10. Work in a group setting with multiple individuals to promote task completion and individualized learning.
  11. Participate in shopping as needed for residents.
  12. Participate in meal preparation following individual meal plans.
  13. Serve as charge technician if assigned including training new staff.
  14. Attend staff meetings in-service trainings and interdisciplinary team meetings.
  15. Ensure all necessary safety and precautionary measures for the well-being of the residents.
  16. Filling out accident/incident reports immediately following an incident according to expectations.
  17. Complete all required service documentation in electronic medical records and/or paper records according to service record documentation standards.
  18. Always maintain a valid drivers license and an acceptable driving record and be responsible for driving community residential vans or other vehicles. Must be at least 21 years of age to drive Center vehicles. Report all traffic citations to the personnel office immediately and in writing.
  19. Routinely lift to 40 pounds independently and routinely lift from 41 to 140 pounds with the assistance of one additional person.

III. Recruitment Standards

  1. Ability to work as part of a team and to handle multiple tasks safely and effectively.
  2. Excellent communication skills.
  3. Establish and maintain an effective working relationship with residents family members team members and community partners.
  4. Learn established policies procedures and practices related to the day supports program.
  5. Demonstrate safe responsible and productive work with minimal supervision.
  6. Adequately follow oral and written instructions.
  7. Routinely lift to 40 pounds independently and routinely lift from 41 to 140 pounds with the assistance of one additional person.
  8. Must have a valid North Carolina drivers license and be insurable by the Centers liability insurance carrier.
  9. Must be at least 18 years of age for employment.
  10. Must be at least 21 years of age to drive an agency vehicle.


Hours/Days of Work (non-exempt)

First shift: 8:00 am to 4:00 pm; MondayFriday

Supervisor Qualified Professional -Community Residential Manager

$15.00 per hr.
