Job Summary:The Early Intervention Specialist provides one-time health education interventions for
individuals groups and communities. This position works collaboratively as part of the
community-based care management entities (CB-CME) team members and families and other
professionals in linkage to care services for members of the community living with human
immunodeficiency virus (HIV) and hepatitis C virus (HCV).
Supervisory Responsibilities: None
Essential Duties/Responsibilities:

  • Refer and link individuals to available prevention services such as PrEP navigation orEarly Intervention Services (EIS)
  • Facilitate referrals to medical care PrEP/PEP navigators and other DAP Healthservices as needed
  • Communicate information consistent with agency standards
  • Provide one-on-one HIV HCV and STI education services to individuals in DAPHealths Sexual Wellness Clinics and community settings with new HIV an assessment to identify social determinants of health barriers and thusutilize prevention strategies for specific infectious and chronic illnesses at time ofindividual visit
  • Provide information and education to each client about prevention tools including properrisk reduction use of condoms lube and antiretroviral medication for prevention (PostExposure Prophylaxis - PEP/ Pre-Exposure Prophylaxis - PrEP)
  • Establish positive community relations
  • Remain well-informed of how to access current community resources to help guideclients through these systems
  • Access patient protected health information as needed
  • Maintain accurate and timely documentation of work
  • Keep Electronic Health Record up to date as needed (ARIES Epic and otherdatabases)
  • Create monthly progress reports
  • Receive referrals from clinicians medical case managers walk-ins and outsidecommunity partners
  • Meet with clients on a drop-in basis
  • Provide client navigation services for clients who are newly Dx HIV or those that havebeen out of HIV care
  • Perform HIV 101 counseling for newly Dx patients and hold space for clients to processtheir new diagnosis
  • Meet with clients after medical appointments
  • Provide food or transportation incentives when completing a qualified HIV medicalappointment
  • Aid with checking in to LabCorp and process of getting labs drawn
  • Assist clients with picking up Rx at pharmacy including contacting clients cliniciandirectly for refills as needed
  • Provide up to date resources on how to enroll into insurance coverage
  • Assist client in applying for sliding fee scale
  • Assist client in enrolling into ADAP Rx coverage and OA HIPP for insurance premiumcoverage
  • Assist client in enrolling in Ryan White Social Services including gathering supportingdocuments such as proof of income proof of residency and proof of insurance asneeded for enrollment
  • Assist in enrolling in Health Care Center
  • Provide navigation services if clients have moved out of the target or who live outside ofthe targeted grant area
  • Provide housing referrals for internal housing department and perform warm handoffs
  • Provide external housing referrals to external housing partners
  • Provide career development referrals with on-site specialist and perform warm handoffs
  • Meet with clients when identified at hospitals or Planned Parenthood for assistance withlinkage to care for HIV and HCV
  • Utilize de-escalation techniques with clients as needed
  • Receive referrals from EIS work queue
  • Complete other related assignments to help meet the goals and objectives of thedepartment
  • Perform other duties as assigned

Qualifications

Required Skills/Abilities:

  • Fluency/Excellent verbal and written communication in English
  • Ability to perform Motivational Interviewing preferred
  • Bilingual in English/Spanish preferred

Education and Experience:

  • Bachelors degree OR experience in medical or health-related fields; social workcounseling nursing public health or other health education
  • Current BLS certification obtained through the American Heart Association or AmericanRed Cross
  • Preferred:
    • The ideal candidate will have an interest in and competence in culturaldifferences HIV/AIDS minority health sexual practices and a demonstratedcompetence in working with the substance abuse community African Americancommunity Latino/a community LGBT communities health care workers youthand/or seniors
    • Knowledge of HIV/AIDS disease Sexually Transmitted Infections (STI)(Gonorrhea Chlamydia and Syphilis) and HCV including diagnosis preventionand risk-reduction strategies
    • Previous professional or volunteer experience working in HIV/HCV/STIprevention or care and/or delivering sexual health education or other behavioralhealth education
    • Understanding of how related chronic and infectious diseases are transmittedand prevented as well as familiarity with Pre-Exposure and Post-ExposureProphylaxis (PrEP and PEP)
    • Knowledge of Stages of Change and behavioral risk reduction

Working Conditions/Physical Requirements:

  • This position is on-site at a DAP Health clinic or location
  • Ability to lift 24 pounds
  • Operates in an office setting at times and requires frequent times of sitting standingrepetitive motion and talking
  • Requires current and valid drivers license and current personal auto insurance
  • Able to travel to DAP Health locations throughout San Diego and Riverside
