We are currently seeking a Caregiver Case Managerto join our assistance services team in Seattle WA.
What will YOU do
The Caregiver Case Manager performs independent comprehensive case management that includes screening and assessing clients; creating implementing and monitoring referrals/care plans; connecting clients to a variety of community resources; and providing client-centered care to help individuals navigate challenging situations. The position may include facilitating caregiver support and skills workshops and groups as well as representing Sound Generations at in-person community events. The position is mostly remote but includes occasional required in-home client visits.
Pay Rate:$31.82 per hour.
Work Schedule:Monday Through Friday 40 hours per week.
