Purpose of the role
To provide non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). Under the Single Aged Care Assessment System Home Support Assessors will work with clients and their representative/s to establish goals specific to the client s needs and develops appropriate strategies and actions including referrals to aged care and non-aged care services.
Requirements
Your key responsibilities
- Provide support services to determine the priority needs of senior Queenslanders including the assessment of aged care requirements and a summary of assessment findings goals and recommendations.
- Undertake Home Support assessments for Commonwealth Home Support Program services to meet the ongoing needs of the older person.
- Identify clients who would benefit from a Reablement Focused Program and action referrals to the appropriate services.
- Develop a client centred Support Plan with the client and /or representative to meet their needs to enable them to live in the community.
