About the role:
We are currently seeking a Full Time Support Coordinator to join our team on a fixed term 12-month basis working 38 hours per week from Monday to Friday. As a Support Coordinator at The Bridge you will assist participants to navigate the NDIS service system using their NDIS funded plans and make informed decisions regarding the provision of resources services and support to fulfil the needs and desires of participants. A Support Coordinator is responsible for:
Using a person centred strength-based service approach to plan develop and coordinate services for the clients in NDIS through the intake assessment process and liaise with internal and external services to maximise participant outcomes;
Planning developing and delivering training regarding internal processes (NDIS portal);
Mentoring and advocating for participants of NDIS;
Monitoring and reviewing service plans and goals in consultation with clients on an ongoing basis;
Providing the appropriate support and assistance to relevant parties;
Establishing and developing alliances with key NDIS and health networks community agencies and services

Selection Criteria:
Experience working with people with a disability from culturally diverse backgrounds.
Experience with and knowledge regarding providing support to people with complex communication abilities and behaviours of concern.
An understanding of the National Disability Insurance Scheme (NDIS) and the disability sector.
A qualification at Certificate IV or Diploma level in a relevant health/social sciences course.
Well-developed communication and interpersonal skills.
Strong emotional intelligence and self awareness
Ability to work cooperatively as a member of a professional team.
Demonstrated ability to work effectively within a culturally diverse group.
Having a keen attention to detail and organisational skills
Well-developed time management skills and the ability to prioritise work.
Demonstrated ability to work autonomously with a high degree of initiative.
Proficient in the use of computer software: Word Excel Internet and email.
Desirable criteria:
Knowledge of and previous experience with the use of client data bases.
Knowledge of community resources/professional services that support individuals in the region.
Previous experience in support coordination in a community setting.
An ability to speak a community language/s.
