ABOUT THE ROLE
As the Support Coordinator you will manage a caseload of NDIS participants providing quality support coordination services to assist participants in understanding their plans navigating their options and connecting and engaging service providers. You support the participants to work towards their personal goals and live more independently. You are the key point of contact supporting informed decision-making and choices based on strong relationships.
