Job Description

Position: Lead ICMS Case Manager

Pay Range: $62000-$70000 Per YR

Reporting To: ICMS Program Manager

Work Type: On-site

POSITION SUMMARY:

The Lead Intensive Case Manager provides comprehensive case management and leadership services within permanent supportive housing to assist clients in achieving the highest level of independence mental physical and emotional well-being. With a whatever it takes for as long as it takes philosophy this position engages even the most service-resistant tenants voluntarily and works collaboratively with program staff and external partners to mitigate tenant issues and prevent eviction.

QUALIFICATIONS:

  1. Bachelors degree in social work Mental Health or related field preferred or five years of case management experience
  2. Minimum one year experience working with the homeless population or two years providing direct mental health or intensive case management services to homeless/chronically homeless individuals
  3. Experience working with clients with mental illness chronic health issues and substance use disorders
  4. Expertise in outreach and engagement strategies intensive case management crisis intervention suicide prevention affordable housing landlord/tenant rights eviction prevention and public benefits applications
  5. Proficient knowledge of managed care organizations Medicare and Medi-Cal
  6. Excellent interpersonal skills with sensitivity to culturally diverse populations
  7. Strong organizational skills and ability to comply with documentation and reporting requirements
  8. Ability to maintain professional boundaries and manage time effectively
  9. Current CPR certification

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The following is a representation of the major duties and responsibilities of this position. The agency will make reasonable accommodations to allow otherwise qualified applicants with disabilities to perform essential functions

  1. Conduct outreach and client engagement
  2. Complete intake assessment and individualized service plans
  3. Assist with rental subsidy applications lease applications housing location and eviction prevention
  4. Manage an active caseload of 510 clients
  5. Provide ongoing onsite intensive case management
  6. Link clients to health mental health and substance use disorder services
  7. Assist clients with benefits establishment
  8. Provide transportation support
  9. Assist clients with life skills community participation volunteer educational and employment opportunities
  10. Conduct individual and small group counseling sessions; facilitate large group sessions as needed
  11. Conduct workshops including relapse prevention self-help skills and money management
  12. Provide ongoing problem-solving support for clients
  13. Provide direct supervision to all DHS staff
  14. Train new and onboarding staff
  15. In the absence of the Program Manager provide overall supervision of the program
  16. Assist with the hiring process
  17. Assist with refining program design and maintaining procedures and systems to ensure quality service delivery
  18. Ensure program services are delivered in accordance with program design and Libertana policies
  19. Coordinate services with outside agencies
  20. Establish revise and implement new policies and procedures in collaboration with the Program Manager
  21. Perform administrative duties with minimal supervision exercising independent discretion and judgment
  22. Requisition office and client supplies maintaining appropriate records and correspondence
  23. Respond to inquiries and resolve operational problems in coordination with the Program Manager
  24. Document progress and all services provided in participants files including employment status counseling sessions case conferences referrals and disciplinary actions
  25. Work effectively with management co-workers participants and others by sharing ideas constructively listening objectively keeping commitments communicating progress and respecting diversity
  26. Maintain confidentiality of client and agency information in accordance with HIPAA regulations
  27. Attend and actively participate in case conferences staff meetings and agency in-services
  28. Follow policies and procedures outlined in the Employee Handbook
  29. Maintain all required credentials and certifications
  30. Report suspected fraud and abuse
  31. Demonstrate knowledge of mandated reporting requirements
  32. Conduct timely documentation of client contact
  33. Attend department meetings (in person by phone or virtually)
  34. Attend all required state-mandated in-service training
  35. Report for work as scheduled
  36. Perform other duties as assigned

Knowledge Skills and Abilities:

  1. Empathy and excellent listening skills
  2. Extensive knowledge of social services welfare legal corrections healthcare substance abuse alcoholism and mental health care systems relevant to the target population
  3. Clear and effective verbal and written communication skills
  4. Ability to advocate successfully on behalf of clients
  5. Ability to travel extensively (50-75% of workday) to provide outreach services

PHYSICAL REQUIREMENTS:

  • Stand sit talk hear and use of hands and fingers to operate computer telephone and keyboard on a frequent basis up to 20% of the time.
  • Reach stoop kneel and bend up to 15% of the time
  • Moderate amount of walking up to 15% of the time.
  • Moderate amount of driving up to 50% of the time.
  • Close vision requirements due to computer work on a frequent basis
  • Light to moderate lifting may be required up to 25lbs on a frequent basis.
  • Pushing and pulling up to 25lbs.
