Position: Lead ICMS Case Manager
Pay Range: $62000-$70000 Per YR
Reporting To: ICMS Program Manager

POSITION SUMMARY:
The Lead Intensive Case Manager provides comprehensive case management and leadership services within permanent supportive housing to assist clients in achieving the highest level of independence mental physical and emotional well-being. With a whatever it takes for as long as it takes philosophy this position engages even the most service-resistant tenants voluntarily and works collaboratively with program staff and external partners to mitigate tenant issues and prevent eviction.
QUALIFICATIONS:
- Bachelors degree in social work Mental Health or related field preferred or five years of case management experience
- Minimum one year experience working with the homeless population or two years providing direct mental health or intensive case management services to homeless/chronically homeless individuals
- Experience working with clients with mental illness chronic health issues and substance use disorders
- Expertise in outreach and engagement strategies intensive case management crisis intervention suicide prevention affordable housing landlord/tenant rights eviction prevention and public benefits applications
- Proficient knowledge of managed care organizations Medicare and Medi-Cal
- Excellent interpersonal skills with sensitivity to culturally diverse populations
- Strong organizational skills and ability to comply with documentation and reporting requirements
- Ability to maintain professional boundaries and manage time effectively
- Current CPR certification
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Conduct outreach and client engagement
- Complete intake assessment and individualized service plans
- Assist with rental subsidy applications lease applications housing location and eviction prevention
- Manage an active caseload of 510 clients
- Provide ongoing onsite intensive case management
- Link clients to health mental health and substance use disorder services
- Assist clients with benefits establishment
- Provide transportation support
- Assist clients with life skills community participation volunteer educational and employment opportunities
- Conduct individual and small group counseling sessions; facilitate large group sessions as needed
- Conduct workshops including relapse prevention self-help skills and money management
- Provide ongoing problem-solving support for clients
- Provide direct supervision to all DHS staff
- Train new and onboarding staff
- In the absence of the Program Manager provide overall supervision of the program
- Assist with the hiring process
- Assist with refining program design and maintaining procedures and systems to ensure quality service delivery
- Ensure program services are delivered in accordance with program design and Libertana policies
- Coordinate services with outside agencies
- Establish revise and implement new policies and procedures in collaboration with the Program Manager
- Perform administrative duties with minimal supervision exercising independent discretion and judgment
- Requisition office and client supplies maintaining appropriate records and correspondence
- Respond to inquiries and resolve operational problems in coordination with the Program Manager
- Document progress and all services provided in participants files including employment status counseling sessions case conferences referrals and disciplinary actions
- Work effectively with management co-workers participants and others by sharing ideas constructively listening objectively keeping commitments communicating progress and respecting diversity
- Maintain confidentiality of client and agency information in accordance with HIPAA regulations
- Attend and actively participate in case conferences staff meetings and agency in-services
- Follow policies and procedures outlined in the Employee Handbook
- Maintain all required credentials and certifications
- Report suspected fraud and abuse
- Demonstrate knowledge of mandated reporting requirements
- Conduct timely documentation of client contact
- Attend department meetings (in person by phone or virtually)
- Attend all required state-mandated in-service training
- Report for work as scheduled
- Perform other duties as assigned
Knowledge Skills and Abilities:
- Empathy and excellent listening skills
- Extensive knowledge of social services welfare legal corrections healthcare substance abuse alcoholism and mental health care systems relevant to the target population
- Clear and effective verbal and written communication skills
- Ability to advocate successfully on behalf of clients
- Ability to travel extensively (50-75% of workday) to provide outreach services
PHYSICAL REQUIREMENTS:
- Stand sit talk hear and use of hands and fingers to operate computer telephone and keyboard on a frequent basis up to 20% of the time.
- Reach stoop kneel and bend up to 15% of the time
- Moderate amount of walking up to 15% of the time.
- Moderate amount of driving up to 50% of the time.
- Close vision requirements due to computer work on a frequent basis
- Light to moderate lifting may be required up to 25lbs on a frequent basis.
- Pushing and pulling up to 25lbs.
