Position: ICMS Case Manager
Pay Range: $24.00-$27.00 PER HR
Reporting To: ICMS Program Manager

POSITION SUMMARY:
This position will deliver intensive case management services to assist clients in achieving the highest level of independence mental physical and emotional well-being. Permanent supportive housing provides services with a whatever it takes for as long as it takes attitude. Participation in services is voluntary; however staff are responsible for finding ways to engage the most service-resistant tenants. The Case Manager will work collaboratively with the Site Manager Clinical Supervisor Director of Services Mental Health Providers and Property Manager to mitigate tenant problems and proactively address issues that put tenants at-risk of eviction.
QUALIFICATIONS:
- Bachelors degree in social work or Mental Health related field preferred.
- Minimum one year experience with the homeless population. Or a minimum of two years of experience providing direct mental health or intensive case management services to homeless/chronically homeless individuals.
- Experience working with clients with mental illness chronic health issues and substance use disorders.
- Expertise in the following areas: chronically homeless individuals outreach and engagement strategies intensive case management services best practice models mental health and substance use disorder services crisis intervention suicide assessment and prevention affordable housing and public benefits applications housing and landlord/tenant rights eviction prevention etc.
- Proficient knowledge of managed care organizations Medicare and Medi-Cal
- Excellent interpersonal skills and sensitivity to working with culturally diverse populations.
- Well-organized and able to comply with documentation and reporting requirements.
- Ability to maintain professional boundaries with clients and effectively manage time.
- Current CPR certification.

- Regularly attend and participates in scheduled case conferences staff meetings and agency in-services.
- Follows policies and procedures as per Employee Handbook.
- Maintains all required credentials up to date.
- Reports Fraud and Abuse.
- Knowledge of mandated reporting.
- Conducts timely recording and/or documentation of client contact.
- Attend Department meetings (in person by phone or on the web).
- Performs other duties as assigned.
PHYSICAL REQUIREMENTS:
- Stand sit talk hear and use of hands and fingers to operate computer telephone and keyboard on a frequent basis up to 20% of the time.
- Reach stoop kneel and bend up to 15% of the time
- Moderate amount of walking up to 15% of the time.
- Moderate amount of driving up to 50% of the time.
- Close vision requirements due to computer work on a frequent basis
- Light to moderate lifting may be required up to 25lbs on a frequent basis.
- Pushing and pulling up to 25lbs.
