Description

About the Role

The Brighter Futures Program also known in the sector as the Extended Post Care Service (EPCS) supports young people transitioning out of the child protection system at the age of 18 across the Brisbane region. The program offers intensive one-on-one case management through an assertive outreach model focusing on enhancing independent living skills for those with complex backgrounds and limited support. Financial support is a key component helping young adults manage the costs of independent living and strengthening their financial decision-making skills in line with their transition goals.

Duties

  • Build strong relationships with clients through effective micro-counselling techniques (body language voice tone active listening) and delivering unconditional positive regard in removing shame or blame perceptions of client beliefs.
  • Deliver immediate safety assessment and safety responses documented and approved by line-Manager in instances of domestic and family violence self-harm/mental health and child abuse.
  • Facilitate ATSICHS Narrative Assessment Tools completed in full by client ensuring the voices of clients are imbedded in their assessment. Facilitate conversations and introspection on all assessment domains.
  • Lead tough conversations with clients outlining non-negotiables of statutory bodies (Child Safety Police Courts Youth Justice etc.). Build change readiness of clients through motivational interviewing strategies and positive reinforcement.
  • Deliver positive reinforcement and continually highlight client strengths and successes to shift/maintain a positive immediate outlook.
  • Deliver ongoing motivational change interventions safely challenging client values with current actions.
  • Build the parenting capability through in-home parental mentoring parenting confidence and attachment knowledge that promote flourishing child rearing.
  • Ensuring all children meet developmental milestones by facilitate family access to ATSICHS paediatric/Mums and Bubs medical services including access to specialist allied health supports.
  • Deliver case managed services according to the program model of care ensuring minimum case load requirements are maintained to ensure equitable access to services of community.
  • Maintain accurate and detailed case notes of all client interactions ensuring case notes are written from the perspective of clients in their voice.
  • Complete a client brokerage plan that is strongly aligned to serious and meaningful goals in the client case plan. Ensure financial support is prioritised on housing transport education employment and emergency relief goals first.

Benefits

  • Permanent full-time position.
  • Starting Salary of $94000 per annum (depending on qualifications and experience).
  • 11.5% Super Not for Profit Salary Packaging (Increases your take home pay).


Skills And Experiences

COVID-19 Requirements

  • Immunisation history showing you have received two doses of a Covid 19 vaccine is encouraged.

  • Bachelors degree in a relevant social services field with at least 4 years case management experience in family support child protection youth support housing or related field.

OR

  • Diploma in relevant Community/Services field with at least 6 years in a case management role in family support child protection your support housing or related field.

OR

  • At least 6 years case management experience in family support child protection youth housing or related field. A commitment to complete Diploma (funded by ATSICHS) while working. And identify as Aboriginal and/or Torres Strait Islander (with confirmation proof) OR non-indigenous and having worked for an Aboriginal Community Controlled Organisation for at least 4 years.
  • Exceptional case management skills including grammatical writing ability personal organisation and time management and execution of services according to the model of care phases of work.
  • Demonstrated ability to create new social work theoretical frameworks of practice aligned with ATSICHS values. Ability to coach other staff on client interventions that build client motivation insight and acceptance self-awareness decision making (agency) and sustainable behaviour change.
  • Ability to apply technical knowledge of relevant legislation (child protection/housing/youth justice etc). Ability to critique and problem solve legislative grey zones while managing organisational and child/client harm risks.
  • Ability to influence decisions of external stakeholders including Child Safety Youth Justice Schools and Hospitals.
  • Ability to connect through systems panel coordination numerous internal ATSICHS programs and services to activate wrap-around care for clients trading-off and negotiating resources across the organisation and between other teams to achieve client impact.

Additional Requirements

  • National Police Certificate.
  • Current Working with Childrens Card (Blue Card) or be eligible to apply.
  • Drivers Licence.
  • Evidence of Work Rights.
  • Travel to work in the community on a regular basis.
