Job Description

Physical Therapist Assistant (PTA)

Medical Home Care and Hospice


Job Objective:

The Physical Therapist Assistant will provide physical therapy services under the direction of a physical therapist in a professional manner in accordance with approved methods and standards of practice adhering to the guidelines of the American Physical Therapy Association


Minimum Qualifications:

Education: Has successfully completed a training program approved by the American Physical Therapy Association. Must meet all requirements for the applicable state regulatory agency. Ability to read write and communicate in English.

Experience: Minimum of (1) years in a home health care agency

Licensure: Certification as required by the State of Connecticut


Essential Job Functions:

  1. Treatment regimens as directed by the physical therapist related to dysfunction of the neuromuscular and musculoskeletal systems.
  2. Treatment may target any of the following areas but may not be limited to strength range of motion mobility balance gait coordination endurance wheelchair use equipment recommendations body awareness orthotic and prosthetic needs body mechanics posture relaxation/stress management vestibular dysfunction cardiovascular condition home exercise program wound care and isokinetic training.
  3. Performs and documents the clients level of function and participation in the development of the total client care plan.
  4. Identifies the client and family needs for home health services and referral for same when needed.
  5. Participation in case management conferences
  6. Instruction of the client family and other agency personnel in the patients treatment regime when indicated.
  7. At least every (30) thirty days will confer with the registered physical therapist and shall document this conference in the clients clinical record. This conference shall include a review of the current care plan and any appropriate modifications to the treatment regime in accordance with changes in the clients status.
  8. Assures quality and safe delivery of therapy services to all clients.
  9. Performs and submits timely documentation on all appointed visits.
  10. Assures compliance with infection control and safety instructions.
  11. Keep current with required CEUs and monitor staff for compliance with required CEUs.
  12. Actively participates in the communication process integral to ensuring quality client care.
  13. Follows established lines of communication and authority.
