Reporting Relationship: Administrator
Primary Social Services Responsibilities:
Responsible for identifying psycho-socialsituations establishing goals for the highest practicable level of well-beingand serving as a liaison between residents families and community resources. Works under minimal supervision of theAdministrator.
Coordinatesadmission process and completesall facility admission paperwork with resident/DPOA/Family. Answers questions familiarizeselder and family with life routines and ensures a smooth transition into thecommunity.
EnsuresAdvance Directives are on file.
CAREassessments/PASRR.
Followup with resident and family after admission to address any concerns and gatheradditional information to assist with Medicaid application as necessary.
Assessand develops a plan of care that meets residents needs. Develops and implements socialcare plans and resident assessments ensuring that social services components ofthe MDS/UDAs are completed and signed on a timely basis. Completessocial history upon admission.
Schedulescare plan meetings in coordination with interdisciplinary team and MDS timing; Providesor arranges for social services for residents/family members as appropriate.
Coordinatesand formulates interdisciplinary discharge plans for residents. Assuressuccessful transfer of residents both within the campus and to outside agencieswithin the community.
Assuresleave of absences are processed smoothly assures all aspects of admissionsprocess of residents is done in compliance with state and federal regulationsand in compliance with Wheat State Manor guidelines.
Completes all documents related to socialwork. Educates families and residents regarding placement care and resident rights.
Provides education to staff on issues relatedto resident care rights and behaviors.
Establishes team spirit between residents andemployees.
Assists with the State Survey process.
Make evening orweekend appointments or home visits if necessary.
Performs additional duties as assigned.
Primary CensusBuilding Responsibilities:
Develop and maintain network with a variety ofreferral sources.
Controllead flow and ensure timely review and decision making by all appropriate teammembers to ensure highest possibility of receiving admission.
Interactwith all leads whether provided through advertising public relationsreferral or personal contact and turn those leads into residents of thecommunity using professional selling skills.
Knowprocedures for lead tracking and keeping accurate records on all leads andprospects.
Create implement and updatetheapprovedcensusbuilding plan.
Professionally represent location in the broadercommunity. Conduct and participate in cultivation events and educationalmeetings public relations programs and special events.
Acts as liaison to family community and socialservice agencies
Participate in civic clubs local committeeschambers etc.; serve on local boards when opportunities present.
Develop expertise of all comparable communitiesin the surrounding area.
Develop advertising communications and publicrelations strategies. Maintain social mediavenues including Facebook Internet web page etc.
Assure compliance with regulatory requirementsrelated to disclosure and marketing materials.
Handleconcerns and/or problems that may arise in the office daily.
Additional Duties:
May not disclose any elder or facilityinformation any system log-in codes or password assigned or permit anyunauthorized access to elder/resident or facility information without expresswritten consent of the facility management.
Coordinates appointments for elders forreferrals of sensory deficits including but exclusive to eye care hearingcare dental care psychiatric care etc.
Provides information to elder and familymembers regarding available services of Long-Term Care State Ombudsman program. Website:
Attends all required facility in-services andeducation.
Assists with coordination of a ResidentCouncil and Family Council and renders assistance to the Councils at thedirection of and with the approval of the Councils. Ensures that all concerns voiced duringCouncil meetings are appropriately addressed and followed up on by theappropriate department representative.
Fosters family and community support of theprogram through newsletters networking programs that bring family members ormembers of the community into the home etc.
Serve as a member of Quality AssurancePerformance Improvement Committee and Performance Improvement Plan meetings.
Serve as a member of the Behavior ManagementRisk and Weight Loss Committees.
Maintain a cleanwell-kept professional appearance as it relates to office and presentation spaces.
Promote Wheat State Manor in a professionalmanner evidenced by appropriate hygiene and clothing.
Willingness to work beyond normal workinghours and in other positions temporarily when necessary
Required Qualifications:
LBSWLMSW or Social Service Designee required
Experienceworking in healthcare and/or senior care industry preferred (AssistedLiving/Memory Care Long-term Care Rehabilitation) but not required.
Mustbe a self-starter and have excellent time management skills and be able to workwithin deadlines.
Excellentcustomer services skills to include ability to maintain a courteous friendlyand helpful attitude with residents visitors and co-workers.
Ability to interact and build relationshipswith older adults and families.
Strong creativity and organizational skills aswell as effective and professional communication skills required.
Computer skills with Microsoft Office (WordExcel and PowerPoint) and ability to learn and utilize Point Click Care (PCC).
Excellent oral and written communication andpresentation skills.
Adherence to State Operation Manual (KDADS)and Power DMS Policy and Procedures.
PhysicalDemands:
Sitting for extendedperiods of time and ability to move throughout the facility and to referralsoutside of the facility.
Ability to readcomputer screens and email mail and communicate via phone.
Ability to lift up to twenty-five (25) pounds.
WorkEnvironment:
Team environment.
Works primarilyindoors in a climate-controlled setting.
