Description
Location: Middletown NY
Pay Rate 52-54k annually
Hours M-F 8-5 with flexibility required
- Perform assessments
- Participate in rotating on-call responsibilities one week per month.
- Complete documentation and reporting in alignment with agency standards.
- Work both independently in the field and collaboratively in a hybrid team office setting.
- Use agency vehicle or personal vehicle (mileage reimbursed) for client transportation and field visits.
- Experience in the provision of Mental Health Services promoting recovery providing crisis management and coordinating and collaborating with community.

- Bachelors degree in Human Services Minimum 2 years experience in providing Mental Health services or;
- Associates degree minimum 5 years experience in human services
- Must be capable to access all rooms in a 2-story home
- Able to work in open space floor plan
- Must be capable to move throughout work day
- Occasional lifting of > 25 pounds
