Under the direction of the Clinical Operations Manager the position will coordinate all aspects of the CTO process including: developing the plan meeting with all parties named in the plan and providing copies to all parties once plan is completed.
RESPONSIBILITIES & DUTIES:
- Organizing the discharge planning activities with the Physician and appropriate community services (Case Management).
- Develop protocols and relationships with local police within hospital emergency and mental health departments and other key groups as needed.
- Be familiar with the Rights advice capacity locally.
- Communicate with other CTO coordinators to discuss issues concerns common problems.
- Work with existing discharge planners crisis and emergency teams and Physicians to delineate roles and responsibilities regarding CTOs.
- In-service sessions: medical staff physicians Home Care other services who receive referrals from hospital potential CTO clients
- Responsible for implementation and management of the service delivery system for CTO as prescribed by the MHA.
- Acts as contact/ referral point for individuals recommended for a CTO.
- Responsible for obtaining/collecting information on referrals to determine eligibility for CTO.
- Assisting psychiatrists with necessary documentation.
- Responsible for writing up the CTO Plan in accordance with all parties named in said plan.
- Responsible for correspondence to all appropriate parties regarding continuation or discontinuation of the CTO
- Responsible for administrative reports regarding CTO statistics and other reporting requirements
- Remain current with changes under the MHA that impact on CTOs.
- Works collaboratively with CTO Case Managers to facilitate implementation and all aspects of the care provision outlined in the CTO.
- Training and education in the local community regarding CTOs.
EDUCATION & SKILLS:
- Requires knowledge and skill acquired through the completion of a University degree in Social Work (B.S.W.) or Nursing (BScN) or a minimum of a Registered Practical Nurse Diploma from a recognized post-secondary institution combined with significant experience working with individuals with complex mental illness and two years of demonstrated working knowledge in the functions of Community Treatment Orders is required.
Registered member in good standing of a professional college.
- Two years demonstrated and comprehensive clinical experience working with individuals with complex mental illness. Two years of demonstrated working knowledge in the functions of Community Treatment Orders is required.
- Comprehensive knowledge of mental illness and treatments; up to date knowledge of relevant mental health policy and legislation including the Mental Health Act; knowledge of community resources; effective oral and written communication skills.
Hours of Work: Thirty-Five (35) hour work week. The agencys regular hours of operation are Monday to Friday 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.
Salary Range: Grade 9 $37.84 $44.81 per hour
Per 2022 - 2026 Collective Agreement
Applicable shift premiums are paid in accordance with the Collective Agreement Article 17.11.
