Flourish Australia has a career development opportunity for a motivated Transition Officer to join our Connect & Thrive program across the Sydney region!
The leave-relief position available is:
- Closed-term full-time from 23/06/2025 - 25/07/2025 (76 hours/fortnight)
- Working Monday to Friday
- Attractive salarystarting at $85000 per annum Super Tax-Free Salary Packaging.
The role has flexibility to be based at any of the following locations however travel may be required between all 5 sites:
- Caringbah
- Kogarah
- Surry Hills
- Maroubra
- Marrickville
About the Opportunity
The Transition Officer works with people accessing the Connect & Thrive program working with people with lived experience of a mental health issue supporting their access to the Connect & Thrive program and in pursuing their recovery goals.
The Connect & Thrive program provides psychosocial support services to assist people who are not receiving psychosocial supports through the NDIS. The program provides individual and group support with a focus on capacity building. As a Transition Officer in the Connect & Thrive Team you will work closely with frontline staff people Flourish Australia supports and key stakeholders to deliver the programs objectives.
To be successful in this role you will have substantial administration experience excellent communication skills be proactive and able to work independently with a thorough understanding of the mental health sector. You will:
- Provide administrative and telephone support
- Facilitate discussions regarding the individual needs and goals of each person accessing the service
- Be proficient in capturing personal information
- Support people to connect with other services as required
- Liaise with local managers
- Assist in the development of reports and projects.

SelectionCriteria
Essential:
- Tertiary qualifications in human services or relevant field or a relevant Certificate IV in mental health related discipline
- Advanced administration skills and at least 2 years experience in a similar role as recognise by Flourish Australia.
- Demonstrated understanding of the challenges faced by people with a lived experience of a mental health issue and how providing a recovery-oriented service is of benefit to people.
- Demonstrated strong time management skills including prioritisation of competing tasks and an ability to work innovatively to solve complex issues.
- Demonstrated strong communication advocacy and interpersonal skills including the ability to liaise effectively with a wide range of stakeholders facilitate outcomes and prepare correspondence & reports in clear and concise language.
- Current unrestricted Australian drivers licence.
Desirable:
- A personal lived experience of a mental health issue or a carer of a person with a lived experience.
