Full Time: Non-Exempt
Work Hours: Monday-Friday: 8:00 am - 4:30 pm. Some evenings may be required as necessary dependent upon agency and client need.
Pay Range: $19.00 - 21.40/hour
Are you passionate about supporting youth with mental health needs Join our dedicated team as a Continuity of Care Community Support Caseworker (CSC) and help empower young individuals to thrive in their communities. This role is ideal for professionals who are compassionate resourceful and committed to making a meaningful impact through community-based mental and behavioral health services.
Deliver Individualized Community Support Services: Provide direct person-centered support to youth enrolled in the CPRC program focusing on emotional behavioral and social development in alignment with their treatment plans.
Manage and Maintain a Caseload: Serve as the primary point of contact for an assigned group of clients ensuring consistent engagement and continuity of care across various settings including home school community and during hospitalizations.
Conduct Clinical Face-to-Face Visits: Meet with clients in person based on clinical necessity which may include daily contact to assess well-being provide support and reinforce treatment goals.
Provide Crisis Intervention and Support: Respond promptly to client crises offering de-escalation safety planning and coordination with emergency services when needed to prevent hospitalization.
Collaborate with Multidisciplinary Teams: Work closely with therapists psychiatrists case managers and external partners (e.g. schools juvenile justice) to ensure integrated care and consistent communication.
Teach Daily Living and Coping Skills: Facilitate skill-building sessions that help youth develop independence in areas such as hygiene time management communication emotional regulation and problem-solving.
Coordinate Community Resources and Referrals: Identify and connect clients and families with appropriate community services including housing education employment healthcare and recreational programs.
Monitor and Document Progress: Track client progress toward treatment goals update service plans as needed and maintain accurate timely documentation in compliance with agency state and federal standards.
Provide Transportation as Needed: Transport clients safely to appointments services or community activities using agency or personal vehicles in accordance with program guidelines.
Support Youth Day Treatment Program: Assist with programming supervision of youth and other activities in the Youth Day Treatment setting when required.
Maintain Professional Standards: Uphold ethical boundaries confidentiality and a positive respectful attitude toward clients families and colleagues.
Stay Current with Required Certifications: Maintain active certification in ProACT and CPR/First Aid (both provided) and participate in ongoing training and professional development.
