We have an exciting opportunity for a Homeless Recovery Coordinatorwithin our RISE Outreach Team who has the skills and experience to assess and engage with our service users ensuring that they access the right intervention at the right time in a way that best meets their needs.

You will work in conjunction with other agencies and focus on engaging with people who have a history of homeless sleeping rough or sofa surfingthat have a substance misuse. You will have some experience of engaging with individuals with multiple and/or complex needs have the ability to build rapport establish boundaries and demonstrate a determined approach to finding solutions.

Our team covers a number of duties alongside caseload management for our service users you will need to be able to be adaptable and willing to be part of a supportive and cohesive team which includes external agencies.

Where: Great Yarmouth Norfolk

Full Time Hours:37.5 per week

Full Time Salary Range: 27861.26 - 32002.35

Contract: Fixed Term until 31st March 2026

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • To establish contact with entrenched homesless drug and/or alcohol users including those unwilling to engage with services providing assertive outreach.
  • To work closely with the drug and alcohol teams and external agencies to ensure that substance misusers and problem drinkers are able to access treatment.
  • To advise service users regarding their substance misuse using recognised theoretical models (e.g. Motivational Interviewing) supporting them to identify and explore concerns before deciding on a possible course of action.
  • To work collaboratively with Mental Health and Child and Adult Safeguarding Agencies to ensure that where appropriate service users have access to address their individual needs.
  • To support service users access Clinical Interventions liaise with Clinical team to establish the best and safest treatment pathway for the service user.
  • Provision of drop-ins at local hostel throughout the city to those housed there.
  • Needle exchange provision at different locations in the city.
  • Caseload management such as regular risk assessments harm reduction advice and reviews provision of psychosocial interventions support clinical interventions cover of duty rota incl. some evening and Saturday work

About you:

  • Good administrative recording and reporting skills.
  • Understanding of the needs of people who are rough sleeping living in emergency accommodation and/or supported housing.
  • In-depth knowledge of at least one of the following: substance misuse alcohol misuse mental health harm reduction NSP provision and delivery of psychosocial interventions.
  • Ability to complete dynamic risk assessments have professional curiosity and build rapport with service users whilst establishingboundaries.
  • Understanding of service users who have multiple and complex needs.
  • Understanding the importance of multi-agency working within a strength based asset building approach and maintaining professional boundaries.
