ESSENTIAL DUTIES FUNCTIONS AND RESPONSIBILITIES
- Directs and plans for a pharmaceutical and clinical pharmacy provision of care.
- Assumes responsibility for the professional actions of pharmacy staff.
- Maintains and improves a comprehensive pharmacy program including clinical pharmacy services medication processing and dispensing medication counseling clinic services inpatient services emergency preparedness and drug information services.
- Provides representation for the pharmacy department at various interdisciplinary team meetings and hospital administrative meetings including but not limited to: Pharmacy and Therapeutics Committee Medical Executive Committee Medication Error Committee and the Center for Medicaid and Medicare Services compliance meetings.
- Provides consultation to local and tribal leaders when requested.
- Reviews current and future equipment needs to maintain compliance with all medication control safety and security regulations.
- Prepares a fiscal year budget including forecast of expenditures for pharmaceuticals equipment contracts equipment replacement/upgrades pharmacy staff and miscellaneous pharmaceutical related device and storage components.
- Ensures compliance with applicable federal state and tribal laws and regulations pertaining to pharmacy services including the U.S. Drug Enforcement Administration United States Pharmacopeia and other regulatory agencies.
- Reviews pharmacy and pharmaceutical related policy annually and is responsible for policy enforcement and/or development as related to pharmaceutical services facility wide.
- Evaluates the impact of pharmacy services through a planned program of quality assurance that meets the standards of the Center for Medicaid and Medicare Services.
- Assumes responsibility for assigning duties to the narcotics control officer as appropriate.
- Assumes responsibility for maintaining all records required by the U.S. Drug Enforcement Administration.
- Assumes responsibility for ensuring monthly CIIV inspections and routine review of controlled substance dispensing and administration in all FDIHB facilities is conducted.
- Performs annual evaluations develop performance improvement plans as needed and review evaluations made by pharmacy supervisors on their direct reports
- Evaluates the need for pharmacy services and streamline processes for efficiency and patient safety.
- Represent the department of pharmaceutical services regarding patient and/or employee complaints.
- Ensures orientation of staff to the functions of the Pharmacy Department
- Ensures formal perception for training programs such as pharmacy student rotations internships COSTEP and residency programs.
- Furnishes the Pharmacy and Therapeutics Committee with accurate drug usage information cost and synopsis of literature.
- Reviews medication records history and physicals inpatient progress notes labs and additional information pertinent to patient care for accurateness efficacy potential drug interactions potential adverse reactions and appropriateness and timeliness of treatment
for patients. - Orders and interprets laboratory tests as appropriate for medication therapy management.
- Adjusts medication based on kinetics reported drug levels weight liver function and kidney function.
- Evaluates and interprets (antimicrobial stewardship) of culture and sensitivity reports and recommendation of appropriate antibiotics.
- Oversees the process of compounding of intravenous medications for use facilitywide. Review appropriateness of intravenous medication; correct dosing compounding and labeling including rate and concentration; and reviews for compatibility with additionally ordered intravenous admixtures when needed.
- Serves as therapeutic consultant and source of drug information for medical staff and nursing.
- Reconciles patient medication records at admission and upon discharge to ensure completeness of medication list.
- Provides technical support for barcode medication administration (BCMA) program and equipment as well as electronic health record (EHR) and computerized physician order entry (CPOE) and Omnicell support.
- Reviews medications ordered by clinic providers patient medication profiles and patient Electronic Health Records to determine appropriateness safety and completeness of medication order authenticity of prescriber possible drug interactions potential for adverse reaction and any other special aspects or considerations in dispensing medications.
- Consults with providers regarding discrepancies found within/between Computerized Physician Order Entry System (CPOE) Electronic Health Record (EHR) and RPMS.
- Reviews the use of nonformulary and/or restricted items for appropriateness and/or approval by the Pharmacy and Therapeutics committee.
- Inspects processed medications for integrity of dosage form; correct labeling handling and storage instructions; and proper medication and quantity as labeled of each prescription before dispensing.
- Inspects bulk prepacked medications before storage.
- Conducts necessary patient education concerning the medication its administration signs of reaction and additional relevant materials per IHS standards concerning patient counseling (i.e. ShowandTell method). Counsel patients on course of treatment and indication for treatment as necessary. When prompted by patients will provide disease state education to further educate patients on management and treatment options.
- Provides drug information to providers nursing family and other healthcare providers utilizing hardcopy online and other drug reference materials as necessary and appropriate.
- Participates with physicians in determining course and preferred disease state
management. - Reviews appropriateness of medication therapy regimen. Advise providers of alternative or preferred medications for costeffectiveness and/or disease state management standards.
- Reviews quarterly antibiograms to ascertain the efficacy of antibiotics versus specified organisms as required to make recommendations.
- Reports and document medication errors and/or interventions per Tshootsoo Medical Center Pharmacy policy.
- Maintains continuing education (CE) requirements of state license(s).
- Maintains Omnicell ScriptPro technology Bar Code Medication Administration (BCMA) carts and support.
- Maintains required certification for specific treatment modalities.
- Performs other duties as assigned.
MINIMUM MANDATORY QUALIFICATIONS
Experience:
- Ten (10) years of direct work experience.
Education:
- Completion of a program an accredited college or University with a Doctor of Pharmacy
(Pharm.D.).
