Schedule: Full Time (40 hours) MondayFriday 8:30am5pm Med/Surg



Job Summary

The Clinical Social Worker (Med/Surg) is responsible for the biopsychosocial assessment possible resources and potential referrals for selected patients in the inpatient medicine/surgery setting. Reporting to the Social Work Manager or a designated supervisor the social worker will provide biopsychosocial assessments and interventions for patients and/or families impacted by a patients acute medical needs and any psychosocial factors or barriers impacting their care. The social worker will identify high risk psychosocial factors impacting a patients health status and as an integral member of the interdisciplinary team will help the hospital staff understand the influence of those factors on the patients course of medical care. The social worker will effectively intervene through evaluation emotional support provision of resources and recommendations and team collaboration around potential barriers to care in the community and possible discharge needs. The social worker will have knowledge of mandated reporting of abuse/neglect trauma informed care cultural and/or age specific concerns and impact unstable housing or homelessness food insecurity substance use disorders psychiatric illness and the various community agencies and resources available. The social worker will participate in departmental and hospitalwide committees as assigned and will collaborate with other disciplines in the hospital.

PRINCIPLE DUTIES AND RESPONSIBILITIES:
Conducts psychosocial assessment of patients to evaluate addiction/behavioral health history/emotional issues/coping style understanding of illness/adjustment/compliance barriers to care cultural issues abuse/neglect and domestic violence.
Meets with patients daily to provide emotional support aftercare planning referrals for programs and community resources. Identifies patients psychosocial financial legal dual diagnoses that effect patient care.
When indicated provides brief family systems work to provide psychoeducation emotional support and guidance around aftercare planning. This includes coordinating family/team meetings as needed and addressing all psychosocial concerns and needs including goals of care if indicated.
Provides mandated assessments when abuse/neglect is suspected (child disabled adult elder or facility) and safety assessment when domestic violence is reported. Files mandated reports if indicated.
Works effectively as part of the interdisciplinary health care team attending daily interdisciplinary rounds communicating regularly with the team and other members on cases and as issues arise.
Documents all timely and relevant assessments and interventions in electronic health record.
Maintains high level of confidentiality for all patients in compliance with HIPAA and Federal guidelines.
Facilitates the appropriate and efficient use of hospital and community resources.
Participates in formal and informal clinical case reviews clinical supervision and educational seminars and other tasks as assigned by direct supervisor or Social Work Manager.

Qualifications

  • Masters Degree in Social Work from an accredited program.
  • Current professional licensure in Massachusetts (LCSW required LICSW preferred).
  • Minimum of one year experience working in a health care setting or related field.
  • Internship/field placements in related health field considered.
  • Knowledge of specific medical/psychiatric illnesses procedures and treatments.
  • Excellent interpersonal skills including negotiation skills necessary to work within a team.
  • Ability to provide rapid clinical psychosocial assessments and interventions with individuals and families
  • Advanced crisis intervention skills.
  • Ability to work effectively as a member of a multidisciplinary team
  • Knowledge of community agencies/resources.
  • Ability to advocate/negotiate systems for/with patients.
  • Tolerance for ambiguity.
  • Cultural sensitivity.
  • Adheres to all C.A.R.E. Standards.
