Job Description

Hesley Group are recruiting for an Advanced Practitionerto work between The Limes in Denaby main and The Paddocks in Wadsworth.

This is a significant position within the service as the Advanced Practitioner will have responsibility to support and develop the operational team in their knowledge and understanding ensuring that the people we support receive a high quality outcomefocusedand personalised service which meets their needs.

Working as part of the local senior management team this role will involve developing and implementing bespoke plans to meet the individual needs of the people we support. This will enable people we supportto have the best possible quality of life and to grow in confidence competence and personal effectiveness.
As part of this role you will be expected to competently use all IT systems deployed by the group with a focus on care and support software and incident management software andensure that accurate records are kept for each individual we support pertaining to the support they receive. Also the managements of incidents with clear follow up including debriefs lessons learnt and continuous improvement feedback.

You will be expected totrain coach and support staff to implement strategies as well as advising on best practice to ensure we meet the individual needs of the people living within the service.
This role does not offer remote working and is not office based . The role requires you to spend time with the staff team working alongside the support staff and team leaders to continue face to face support to help the people we support achieve their aspirations and goals.
This role is 40 hours a week usually Monday to Friday. However you will be required to work some weekends and late shifts to meet the needs of the two services.

About You

We are looking for someone who has a successfultrack record of working with adults with learning disabilities and AutismSpectrum Disorder as well as behaviours of concern.
You will be a positive role model and coach for the team you are supporting as well as being self directing with excellent organisational skills who is reliable and able to identify and see tasks through to completion.
We are looking for someone who is committed and enthusiastic about the people we support achieving the best possible outcome and have a great quality of life.
For this role it will require you to have a full UK Driving license and access to your own vehicle as you will need to respond between the two services.
