Primary Objectives
The Behavioral Health Integration Specialist is a professional staff member that is a core member of a collaborative care team. This job description is not an allinclusive list of the duties and responsibilities of this position. PCI Employees are expected to perform all duties and responsibilities necessary to meet the goals and objectives of applicable programs and departmental objectives as assigned.
Professional Staff of the Poarch Creek Indians will consistently demonstrate the abilities to influence innovate flex their style and problem solve along with a strong commitment to embodying the core values of the Poarch Creek Indians which include Perseverance Opportunity Accountability Respect Culture and Honesty.

Essential Functions
  • Completes all Documentation (notes billing calls etc.) in Electronic Health Record in a time frame of five (5) days.
  • Provides assessments crisis management and trauma informed care services to Primary Care patients and their families on an individual or group basis in coordination with the medical provider.
  • Provides direct screenings for common mental health and substance abuse disorders. Facilitate patient engagement and follow up care.
  • Interprets patients resources and case management needs treatment record and coordinates between providers nurses and behavioral health to determine interventions to address in gap in care.
  • Assists medical and nursing staff as needed with coordination of care and disposition of patients from Primary Care to patients requiring placement or transfer to mental health/ substance abuse facilities.
  • Provides patient education about common mental health and substance abuse disorders and the available treatment options.
  • Systematically tracks treatment response and monitor patients for change in clinical symptoms and treatment side effects or complications. Facilitates treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or specialized mental health care.
  • Routinely provides patient education regarding the connections between mental and emotional issues and the possible physical manifestations.
  • Supports psychotropic medication management as prescribed by medical providers focusing on treatment adherence monitoring side effects and effectiveness of treatment.
  • Monitor and assess current screening measures utilized by Behavioral Health and Primary Care for effectiveness
  • Participates in Government Performance and Results Act (GPRA) Team.
  • Monitors Behavioral Health clients by taking vitals completing dictation completing chart reviews triage sheets referrals and prior authorizations.
Job Requirements
  • Bachelors Degree in Sociology Psychology Social Work or related field required.
  • Minimum of five (5) years of employment experience in a counseling role within a behavioral health setting required.
  • Must maintain current professional licensure in Basic Life Support (BLS) or obtain licensure within ninety (90) days of official date of hire in this position.
  • Must possess certification of Mandatory Reporter Training or obtain certification during the first week of employment.
  • Ability to work odd and irregular hours as needed.
  • Must successfully pass the required criminal and character background check.
  • Must possess a valid state drivers license and insurable driving record according to Tribal insurance guidelines.
  • Ability to travel and participate in required training leadership development and other events.
  • Ability to perform all duties and responsibilities of this position adequately and successfully.

Compensation and Benefits
  • The starting pay will depend on factors such as experience level and skillset.
