Pay: $40.00 $52.00hourly!

Chronic Care Coordinator Job Details:
- Parttime or Fulltime direct hire opportunity
- Setting: Clinic
- Shift: very flexible two days if part time negotiable for full time hours
- Chronic Care Coordinator will coordinate care and facilitate communication between healthcare providers specialists and community resources to ensure continuity of care. Advocate for patient s needs and rights addressing barriers to access healthcare services and promoting patientcentered care. Utilize evidencebased practices and clinical guidelines to deliver highquality costeffective care and achieve optimal patient outcomes.
- Chronic Care Coordinator will perform Annual Medicare Wellness Visits in conjunction with providers. Document patient assessments interventions and outcomes accurately and timely in electronic health records (EHR) or other designated systems. Participate in quality improvement initiatives research projects and professional development activities to enhance clinical practice and promote innovation in chronic care management.
Chronic Care Coordinator Requirements:
- RN License
- Associate Degree in Nursing
- At least one year of working experience in a critical access hospital setting.
- BLS certification
