Job Description

Concern Housing is seeking an Addiction Specialist who will be responsible for providing case management and chemical dependency counseling services directly to clients receiving Mobile Residential Support Team services. The Addiction Speciliast is considered a resource in that they should be familiar with community resources and services to which they may help their clients gain access and also be an advocate for their clients.

Position: Addiction Specialist
Schedule: Monday Friday; 9am 5pm
Location: Middle Island NY
Salary: $57500

Responsibilities Include but are not limited to:

Provision of chemical dependency counseling services to assist individuals to develop the skills needed to prevent relapse promote recovery and independence and increase selfsufficiency. Counseling shall include assessment of needs identifying triggers increasing coping skills development of resources service planning referral to necessary treatment providers case coordination and collaboration community integration supportive counseling and crisis intervention.
Provision of case management services (assistance in forming linkages and/or referrals to clinical services day programs medication management life skills counseling services vocational services and opportunities entitlements etc.
Timely submission of all documentation and maintenance of all client charts in compliance with agency standards.
Attendance at supervision and staff meetings.
Other duties as assigned by the Director of Supported Housing.
Overall assessment of client welfare and appropriate intervention and followup as needed.
Remain current with all Agency required compliance documents and trainings.

Qualifications: CASAC or CASACT credential is required for this position. Bachelors Degree in Psychology Rehabilitation Social Work or other related discipline plus 2 years experience working in a similar setting and/or working with people in recovery from chemical dependence. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
