Croydon Council Adults Social Care is seeking to recruit a Health and Wellbeing Officer to work in the Adults Early Intervention Support Service (AEISS) leading the way in delivering vital firstpointofcontact services to adults in need of support.
Key responsibilities include ensuring timely and efficient responses to referrals and inquiries received. You will respond to phone calls and incoming referrals ensuring that individuals receive the right care and advice promptly. You will have a customerfocused approach focusing on early intervention prevention and supporting independence. The role requires that you ensure that individuals carers and families receive clear and accessible information and advice. You will work closely with individuals and communities to ensure that services are responsive and inclusive particularly for vulnerable or hardtoreach groups.
