Job Description

Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness poverty and addictions. We hire people that are skilled and passionate to meet our mission to end homelessness through outcomebased strategies which support personal and community transformation.

The Housing Case Manager works as a member of a multidisciplinary behavioral health care team providing integrated behavioral health services to clients seen at Old Town Recovery Center. The Housing Case Manager is responsible for providing housing support the OTRC mental health teams ensuring care that is accountable comprehensive proactive wellcoordinated and patientcentered.

The programs at Old Town Recovery Center (OTRC) address all treatment needs including crisis management substance abuse treatment psychiatric evaluation and treatment physical health treatment mental health treatment accessing benefits and entitlements accessing appropriate and affordable housing. The programs are responsible for proactively managing the healthcare needs of the clients on their assigned team including medical mental health addiction and health behavior needs.

Schedule: MondayFriday 8:30am5:00pm

Compensation: $23.69/hour

Union Seniority Bid Ends: 3/21/25

MINIMUM QUALIFICATIONS:

  • Must be registered with the appropriate Oregon state licensing board to meet at minimum QMHAR requirements.
  • Bachelors degree in Behavioral Sciences field or a combination of at least three years work education training or experience in chemical dependency and/or mental health treatment;
  • Has the competencies necessary to:
    • Communicate effectively;
    • Understand addictions and mental health treatment and service terminology and to apply the concepts;
    • Provide psychosocial skills development; and
    • Implement interventions prescribed on a treatment plan.
  • Experience with severe and persistent mental illness (SPMI) preferred.
  • Bilingual (English/Spanish) preferred.
  • Must meet CCC privileging requirements as required by FTCA.
  • Must have current CPR certification prior to start date.
  • Must pass a preemployment drug screen TB test and background check. This includes clearance by the DHS Background Check Unit.
  • Will be required to carry an agency cell phone for work use. Cell phones will be provided by Central City Concern.
  • If using a vehicle for agency use you must possess a current drivers license access to a vehicle and qualify as an Acceptable Driver as designated in Central City Concerns Fleet Safety policy. Must pass an initial driver training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100000/$300000 personal auto liability coverage.
  • Must have the ability to complete forms thoroughly accurately and with legible handwriting.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Pursue affordable housing options for clients both inside and outside the Central City Concern system.
  • Assist clients with accessing affordable housing both inside and outside the Central City Concern system.
  • Assist clients in completing all appropriate forms for obtaining housing.
  • Attend all meetings required in order to best work with landlords resident services and building staff.
  • Assist clients with developing working relationships with landlords resident services and building staff.
  • Attend required meetings and trainings.
  • Advocate for clients when necessary; attending meetings such as landlord tenant or court with client.
  • Help clients move or make improvements to their dwelling to insure housing stability.
  • Must maintain client records as required by program standards and funding requirements including documentation of clinical/case management interactions.
  • Submit billing for clinical interactions if credentialed as a QMHA.
  • Conduct Rent Well Classes
  • Adhere to all state and federal privacy regulations including HIPAA and 42 CFR Part 2 and to CCC policies and agreements regarding confidentiality privacy and security. Support compliance with all privacy and security requirements pursuant to community partners and outside providers patient confidentiality agreements including privacy and security
  • requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department as well as to your supervisor or their designee.
  • Perform other duties as assigned.

  • Knowledge of deescalation methods or ability to be trained in deescalation methods.
  • Ability to learn and apply training instruction.
  • Ability to take directions from supervisor and work amicable with peers in a health care setting.
  • Professional maintenance of appropriate boundaries.
  • Knowledge of landlord tenant law Permanent Supported Housing Shelter Plus Care voucher system Section 8 and any other programs as deemed necessary.
  • Ability to work effectively with peers individual clients and groups.
  • Ability to work as a team member remain flexible and to establish priorities as a group.
  • Ability to apply principles learned to new situations.
  • Ability to maintain accurate records and necessary paperwork.
  • Ability to communicate clearly and concisely both orally and in writing.
