Job Description

Job Title: Housing Support Manager

Reports to: Program Manager Supportive Services

Locations: Denver

Compensation: $58000$65000 generous benefits including 401k matching 100 employerpaid premiums for health dental vision; paid vacation paid holidays; $40 cell phone stipend


Colorado Village Collaborative seeks a Housing Support Manager with experience supervising a case management team. The Housing Support Manager oversees the operation of the Case Management team to assist with client support.


The Housing Support Manager reports directly to the Program Manager Supportive Services and provides direct supervision and oversight of the Case Managers (CMs) who function within all of the CVC site locations. The Manager is responsible for operationalizing Case Management workflow standards and processes.


Schedule / Hours:

  • 40 hrs/wk flexible with some nights and weekends may be needed
  • Additional time will be spent in organizational meetings meeting with external partners and coalitions meeting with MicroCommunity community members etc.


Key Area # 1 Case Management Team Management

  • Supervise Case Management staff and their responsibilities according to the following Key Areas.
  • Maintain Scheduling.
  • Ensure accountability and follow through on Case Management team and sitespecific policies and procedure
  • Report accountability and noncompliance to the Program Manager and Assistant Director of Supportive Services


Key Area #2 Oversee One to One Case Management Team

  • Provides support to the Case Management team.
  • Functions as a departmental subject matter expert.
  • In conjunction with the Assistant Director the Site Manager will assess the daily census assignment ensuring acuity and caseloads are equitable
  • Provides handson support for critical staffing needs.
  • Monitors productivity.
  • Development and oversight of CM work schedules/calendar management
  • Promotes an environment of interdepartmental continuum of care and post acute care collaboration.
  • Will participate in the development and presentation of projects and education topics to designated departments and other others as needed.
  • Provide education and feedback to provider groups as indicated.
  • Monitors monthly reporting to Program Directors..
  • Participates in the review and updating of Case Management policies.
  • Escalates and keeps Program Directors informed of workflow or personnel concerns.


Key Area #3 Data Collection and Documentation

  • Assists with the preparation of grant specific midyear learnings calls and yearend reporting.
  • Oversees the maintenance of confidentiality of all client information.
  • Ensures all necessary documentation including HMIS is completed in a timely manner.
  • Completes all required forms and reports in a complete and timely manner.


Key Area #4 Professionalism and Teamwork

  • Conducts Case Management team meetings
  • Understands and follows the related policies and procedures of the MC operations
  • Attends required meetings and regular supervision focusing on the key areas above.
  • Attends and participates in formal and informal staff meetings and addresses client and program issues
  • Provides excellent internal and external customer service and communicates in a professional timely and effective manner
  • Actively participates collaborates and contributes to the multidisciplinary team to promote a team culture in which each clients point of view experiences and preferences are recognized understood and respected and in which client selfdetermination and decisionmaking in service planning are maximized and supported.


Other Responsibilities:

  • Complete other special duties and tasks as assigned by management

Physical Requirements:

The position requires the ability to perform the essential functions with or without reasonable accommodation. Tasks may involve prolonged periods of standing sitting or walking. Additional requirements include occasional reaching bending crouching or kneeling along with manual dexterity to operate tools equipment and technology. Visual and auditory ability to respond to tasks and ensure safety are also essential.

Requirements

Qualifications:

  • High School Diploma or equivalent and one year either working with unhoused individuals in nonprofit or supervising others is required. Additional education or experience in social work human services or related fields preferred.
  • Reliable transportation is required
  • Cell phone to download and access CVC applications is required
