Job Description
Under the direction of the Space Strategy & Planning Coordinator responsibilities include: Ensure full participation with Federal and Municipal entitlement programs for all clients. Support client housing need and services as IAP defined. Maintain working relationship with community housing resources. Maintain documentation and compliance of the HUD contract. Requires access to personal vehicle and valid drivers license for client transportation.
- Works with ACCS staff and clients in order to promote Housing First model
- Maintains positive active relationships with local landlords housing authorities and other housing resources in order to support the needs of clients in ACCS services
- Actively works with Department of Mental Health on establishing best practices for housing
- Meets individually and in groups to support clients on housing needs such as filling out paperwork and doing searches
- Keeps current on all changes/trends in housing services/subsidies
- Works with business office to maintain current records on all subsidies
- Obtains MAP certification within 6 months from date of hire. Administers medication and completes documentation in accordance with MAP policy.
