Typical day as our Support Coordinator
- Meeting with customers to create support plans and determine their support needs. (Our customers are Single Homeless Men with complex needs)
- Supporting customers with daily living activities such as preparing meals shopping budgeting maintaining their tenancy and moving onto independent accommodation.
- Carrying out risk assessments support planning goal setting and regular reviews.
- Cuppas chats and catch ups. Theres no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.
- Facilitating property maintenance and Health and Safety checks.

You bring
- Passion to support our customers to live their best life working collaboratively with an eye for detail.
- Experience of creating personcentred support plans and working as a team to accomplish Customers goals.
- Experience of coordinating and assessing customer referrals.
- The ability to workon your own initiative remaincalm under pressure and havea resilient approach

Job details
- We are as flexible as we can be to support our colleagues. Where were flexible with you we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- We work on a rota basis Monday to Sunday between the hours of 7am & 7pm. This includes Bank Holidays and Christmas.
- You must be able to use technology for creating and updating support plans complete online learning and to collaborate with colleagues.
- Youll need an Enhanced DBS check done and we pay for that.
