Job Description
InGenesis is seeking an LVN Wellness Case Manager to work with our client. In this role you will evaluate plan and provide case management services health education and counseling services for patients with complex chronic conditions such as but not limited to Diabetes Hypertension Congestive Heart Failure and Asthma following BACH s Chronic Care model.

Essential Responsibilities
- Performs a variety of complex professional analytical and confidential case management services.
- Maintains Chronic disease registry and enter tracks and updates patient information.
- Makes appointments and sends reminders of regular and ancillary visits and preventive care.
- Reviews registry for overdue lab work.
- Uses registry to target patients for chronic disease care management activities such as making sure patients come in for regular visits preventive care actions that are overdue and other triggers for panel manager attention.
- Develops and implements a mechanism for tracking referrals and ensuring clients successfully followup on referrals according to timing protocols that are implemented.
- Assists with eligibility determination to ensure access to services.
- Conducts messenger activities (e.g. delivers brief message about refills or appointment scheduling reminds patients to bring medication bottles to all visits.
- Maintains regular communication with other clinical staff involved in panel management.
- To support patient and client objectives you may be required to work at other BACH locations within a 50mile radius.
- Performs other duties as assigned.
Minimum Qualifications
- High School Diploma.
- Completion of LVN program.
- Current license to practice as an LVN in California.
- 12 years of workrelated experience in a medical environment.
- Active phlebotomy certificate/blood draw certification.
- Access to reliable transportation.
- Proficiency in Microsoft Office and the internet is a MUST.
Preferred education experience training
- Associate or bachelors degree in related field.
- Experience in a community health center longterm care facility or public health setting.
- Experience working with patients with Chronic Diseases; Diabetes and Hypertension.
- Outreach and presentation experience
Physical Demands:
This position requires:
- Sitting walking and standing associated with a normal medical/clinical office environment; some bending stretching and lifting.
- Manual dexterity used to include but not limited to standard office equipment and computer keyboard.
- Traveling to work sites includes some exposure to outside elements.
- Presenting information in front of a group.
- Reading and writing in order to review records for accuracy.
