Trimed HealthCare invites you to join our team of compassionate care givers as a Aging Waiver Support Coordinator and earn a competitive salary. Requires at least 1 year case management experience / supports coordination in the PA State Home and Community Based Waiver Services. Bachelor Degree level applicant. Experience in SAMS and HCSIS.
