Rate: 32 per hour
An exciting job role has recently become available in Barnet. We are looking for a new member for their Hospital Team.
What will your responsibilities be
You will educate patients and their families about illnesses treatment plans and available resources. You will conduct psychosocial assessments to identify any mental or emotional distress and address these issues. You will provide counseling to patients and families in crisis or experiencing distress. You will develop and implement individualized care plans to ensure safe and effective discharge from the hospital. You will work with interdisciplinary teams including doctors nurses and case managers to coordinate patient care and ensure seamless transitions. Additionally you will advocate for the best interests and wishes of the patients ensuring their voices are heard in care decisions.

Qualifications and Experiences:
You must have Social Work England registration and be eligible to work in the UK. Additionally you need to hold a full UK driving licence and possess postqualified experience. A degree or equivalent qualification in Social Work is also required.
