Job Description
- Assess implement and monitor patients mental health needs including conditions such as ADHD anxiety and PTSD
- Conduct mental health evaluations & obtain complete medical histories from patients and families
- Provide counseling education & support for patients and families managing mental health conditions
- Collaborate with physicians & the healthcare team in a clinical team-based setting to ensure coordinated care
- Refer patients to psychiatric providers or primary care physicians for advanced or complex mental health care
- Perform behavioral health screenings using evidence-based tools such as Conners BECK DSM Level 2 ASQ-3 and M-CHAT based on age and need
- Implement and support Patient-Centered Medical Home (PCMH) standards including motivational interviewing self-management coaching and population health management
- Provide training to staff in motivational coaching & behavioral health techniques
- Assist patients in navigating the healthcare system and accessing appropriate services
- Maintain accurate up-to-date patient records in compliance with regulatory and collaborative care standards
- 10 Paid Holidays per year
- PTO 6.25 hours per pay period (20.31 days yr)

- Retirement - employer contribution min 2% of gross after 1 year & up to 4% of gross after one year
