Location:Field-Based with Office Support - Serving 13 Counties in Georgia Around the Metro Atlanta Area
Status:Full-Time / Part-Time
Job Summary:
Our Client is hiring for a Hospice Social Worker! The Hospice LMSW provides medical social services to terminally ill patients and their families in accordance with the interdisciplinary plan of care. Utilizing the Homecare Homebase (HCHB) electronic health record system the LMSW supports patients emotional psychosocial and resource needs while promoting dignity quality of life and patient-centered goals during end-of-life care. This position requires travel to all 13 counties served by the agency. If you do not like to travel or visit patients in their place of residence this job will not be a good fit.
Essential Duties and Responsibilities:- Conduct initial and ongoing psychosocial assessments using HCHB within the required documentation timeframe.
- Develop and implement individualized social work care plans in collaboration with the IDG team.
- Provide emotional support counseling and education to patients and families related to end-of-life issues grief family dynamics and adjustment.
- Assist families with advanced care planning community resources funeral planning guardianship and legal/financial concerns.
- Participate in interdisciplinary team (IDG) meetings presenting findings and recommendations.
- Document and attend IDG meetings as required per CMS Hospice Conditions of Participation (CoPs).
- Document all visits interventions and communications accurately and timely in HCHB.
- Assist with crisis intervention and provide bereavement support as part of the overall hospice program.
- Advocate for patient rights wishes and preferences.
- Maintain up-to-date knowledge of Medicare/Medicaid regulations and community resources.
- Ensure HIPAA compliance and uphold all hospice policies and procedures.
- Masters Degree in Social Work (MSW) from an accredited school.
- Current Georgia LMSW license in good standing.
- Two (2) years of hospice or healthcare experience preferred.
- Proficiency in using HCHB (Homecare Homebase) EHR or similar clinical systems.
- Strong interpersonal communication and time-management skills.
- Ability to work independently in a field-based setting with flexibility and compassion.
- Must be able to travel to patient residences ALFs SNFs or hospitals within the service area.
- Must have reliable transportation valid drivers license and auto insurance.
- May involve exposure to patients with terminal conditions in home or facility environments.
- Must be able and willing to travel throughout all 13 counties serviced by the hospice agency.
- Experience with terminal illness palliative care or grief counseling.
- Familiarity with Medicare hospice Conditions of Participation (CoPs).
- Comfort navigating electronic records and mobile charting using HCHB tablets.
- Required 2yrs of Hospice experience
