Summary

The Integrated Care Coordinator (ICC) is a patientfacing care coordinator for dental services. This role provides patients with direction health education insurance coverage information and support in navigating/accessing integrated health services within IBCC. This role works closely with the Dental Director Dental Clinic Coordinator Patient Service Representatives (registration) Outreach & Enrollment Workers (benefits assistance) Referrals Specialists and other IBCC team members to assure patients receive timely comprehensive care. The ICC is dentalcentric and possessing understanding of how oral health is connected to the rest of the body. They serve as a source of support for dental patients who require warm handoffs between care services to assure high quality care and patient stabilization. They also reach out to patients being seen in other departments to establish dental care. The ICC outreaches and engages all lBCC patients to reduce gaps in care with a particular focus on senior health care. They also facilitate medical consults of patients for the dental team when needed. The ICC is selfmotivated and takes initiative to approach patients and providers to secure services that aid in the goal of integrating all lBCC services to all patients with priority focus for seniors (ages 60.

Essential Functions

Provides care management and health care navigation services within a Federally Qualified Health Center (IBCC) serving individuals and/or families.

Uses strong customer service skills to perform outreach and engagement inperson and via phone assessment/evaluation of needs provides education meets and/or distributes materials to patients coordinates and schedules services and/or performs warm handoffs to needed services.

Will serves as a representative of IBCC of the Senior Oral Health Partnership (SOHP) and will attend meetings coordinate activities and report to the SOHP in collaboration with the Dental Director. Flexible to meet with community partners and participate in meetings as needed to meet funder requirements.

Possesses strong organization prioritization and time management skills as well as ability to work autonomously.

Ability to review communications and respond on multiple platforms checking each multiple times per session.

Verifies insurance eligibility for services as appropriate and facilitates meetings for patients to apply for insurance with eligibility and enrollment specialists.

Participates as a member of a multidisciplinary team. Effectively communicates and collaborates with patients other health professionals clinical staff and colleagues.

Participates in monthly outreach events in or around the IBCC.

Participates in meetings presentations workgroups and/or learning collaboratives as assigned and attends the weekly clinical coordination meeting.

Searches the patient database to identify seniors who may need navigation of services and help to facilitate such.

Works with health care providers to facilitate services to patients from dental to other IBCC departments and vice versa.

Serves as the goto person for dentalrelated matters for scheduling and coordinating care between the dental clinic and the other departments within the organization.

Maintains records of interactions and outcomes of efforts to coordinate care.

On time completion of assigned trainings and policies.

Performs other duties as assigned

Additional Essential Qualities

The ICC is responsible for scheduling organizing and managing multiple aspects of a patients healthcare maintenance and treatment.

The ICC employs strong organizational skills necessary to systematize a care plan for several patients at once while coordinating with diverse healthcare workers who hold varied schedules.

The ICC has professional verbal and written communication skills that expedite and facilitate patient understanding of health needs while maintaining a high level of respect and confidentiality for the patient.

Qualifications

High School Diploma or GED. Bachelors degree preferred in related field.

Minimum of four years of experience in the healthcare industry with a specific focus on working

Directly with medical patients.

Bilingual (Spanish/English) written and verbal required.

Dental Assistant experience preferred but will train on the job for the right person.

Experience with electronic health record systems. Basic user of MS Office and other essential computer skills.

Current American Heart Association healthcare provider CPR (BLS) certification.

Experience supporting individuals with disabilities agerelated conditions mental illnesses or challenging behaviors highly preferred.

Knowledge of MediCal Medicare Managed Care Plans and insurance guidelines preferred.
