Job Description
PURPOSE STATEMENT:
Plan direct define and coordinate individualized and group therapeutic support and services to adults with mental health behavioral health and substance abuse issues promoting recovery symptom reduction increase coping skills and the achievement of the highest level of social functioning.
ESSENTIAL FUNCTIONS:
Conduct a psychosocial assessment of the patient and update as needed.
Participate in a multidisciplinary treatment team meeting to discuss psychosocial assessment treatment planning and ongoing discharge planning.
Develop effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient.
Communicate or aid family members in understanding dealing with or supporting the patient.
Manage a caseload and assume responsibility for patient chart documentation.
May facilitate education support groups and referrals as requested.
OTHER FUNCTIONS:
Perform other functions and tasks as assigned.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
Bachelors degree in Social Work from an accredited college or university including new graduate. 12 years experience working with patient population preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
Eligible for state licensure as one of the following: Licensed Social Worker (LSW) Licensed Associate Counselor (LAC) Certified Addictions Counselor (CAC) Registered Addiction Counselor (RAC) as required by the state in which the facility is operating.
First Aid CPR deescalation and restraint certification required (training available upon hire and offered by facility).
