Job Description

We are working closely alongside a Local Authority in Liverpool to assist with the appointment of a Social care Assessor to work in a residential care setting.

We have both regular shifts (35 hours per week) and bank shifts available. Please note that we dont offer sponsorship and the worker should be able to work full time without restrictions. We would need 5 years work references checked out before you start the shifts.

Rate of Pay: 17.80 - 23.49 per hour

Summary:

Liverpool City Council is seeking a dedicated and empathetic Social Care Assessor to support the delivery of high-quality adult social care services. The post holder will play a key role in assessing the needs of adults and carers identifying appropriate support plans and working collaboratively with individuals families and professionals to promote independence and wellbeing in line with the Care Act 2014.

Responsibilities:

  • Undertake person-centred assessments of adults and carers in line with statutory duties and council procedures.
  • Work with individuals to co-produce care and support plans that promote independence and meet eligible needs.
  • Provide information and advice to individuals and families about services support and community resources available.
  • Liaise with professionals from health housing and voluntary sectors to ensure effective multi-agency working.
  • Support safeguarding procedures by identifying and reporting concerns about abuse or neglect.

Qualifications:

  • NVQ Level 3 in Health and Social Care or equivalent qualification.
  • Evidence of ongoing professional development.

Essentials:

  • Experience in working with adults in a health or social care setting.
  • Experience in carrying out needs-led assessments and support planning.
  • Demonstrable understanding of the Care Act 2014 and eligibility criteria.

Please note:

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires an Enhanced DBS
