Job Summary
Bolton Clarke Groupone of Australias largest independent not-for-profit providers of home care retirement living and residential aged care operating nationally and Clarke Residential group now has 88 Aged Care Homes across QLD NSW VIC SA & WA with the inclusion of the Allity McKenzie & Acacia Living Group.
The Care Partner is the designated care manager under the Support at Home Program responsible fordelivering care management activities that enable older people to live safely and independently athome.You will play a key role in planning and overseeing care service delivery across all ongoingsupport classifications within scope and work closely with the appropriate clinical team memberswhen a participants needs require clinical input or complex care planning.
As a Clinical Care Partner you will:
- Facilitate or participate in case conferences ensuring care plans reflect shared decision-makingand supported risk-taking.
- Collaborate with participants to develop individualised care plans.
- Liaise with general practitioners specialists and allied health providers to ensure integratedcare.
- Review care notes and service delivery data; proactively identify and address emerging risks orchanges in participant needs.
- Consistently meet quality compliance and audit requirements ensuring accurate and up-to-date recordsare maintained.
- Collaborate with participants to plan manage and understand individualised quarterly budgetsin line with their care plan.
- Monitor spending to prevent underspends and overspends and align resource allocation withchanging needs.
- Maintain accurate timely care notes and ensure documentation supports outcomes under theStrengthened Aged Care Quality Standards.
- Ensure care management claims comply with Support at Home business rules includingexclusion of administrative rostering or travel tasks.
- Supporting participants (and their carer or supporter) to make informed service decisions.
To be successful you will have:
- Cert III Individual Support or Diploma-level qualification in Nursing Community ServicesAged Care Case Management or equivalent experience.
- Demonstrated experience in care coordination case management or similar roles in aged carehealth or disability services.
- Strong understanding of the Support at Home Program Quality Standards and aged care rights.
- Strong communication empathy and cultural competence.
- Competence in interpreting clinical guidance and escalating appropriately.
