Job Title: Registered Nurse Case Manager
Location: Eureka CA
Type: Contract
Pay Rate:
Local: Up to $2400 Weekly
Traveler: Up to $2800 Weekly (Stipend Included)
Duration: 13 Weeks
Shift & Schedule: Day Shift 5x8 Hours (08:00 AM 04:30 PM) 40 Hours Weekly Every Other Weekend Required

Position Overview:
Navitas Healthcare LLC is seeking Registered Nurse Case Manager for an exciting Travel or Local job in Eureka CA.

Primary Responsibilities:
Coordinate and manage patient care plans across the continuum of care
Perform discharge planning and utilization management
Collaborate with physicians nursing staff and interdisciplinary teams
Assess patient needs and facilitate appropriate resource allocation
Maintain accurate documentation within Epic EMR system
Support patient advocacy and continuity of care initiatives
Float between like units based on facility needs

Requirements:
Active RN License required
Minimum 2 years of Case Management experience required
Acute care hospital experience required
Experience with Epic charting preferred
Strong communication and care coordination skills
First-time travelers accepted with clinical approval
Associate Degree or BSN from an accredited institution required
BLS Certification required

No on-call requirement
Locals accepted
Travelers may float between like units as needed
