Contract: 13 weeks
Schedule: Tuesday Saturday 8:00 AM 4:00 PM
Guaranteed Hours: 37 hours per week
Experience: Minimum 2 years required
About the Role
We are seeking an experienced Hospice Registered Nurse for a 13-week quick-start contract opportunity in Rutland Vermont. This role serves as a hospice case manager providing comprehensive patient assessments individualized care planning symptom management patient and caregiver education and coordination with the interdisciplinary care team.
Responsibilities
Provide hospice nursing care and comprehensive assessments for assigned patients
Develop update and communicate individualized plans of care with patients caregivers and the IDT
Manage caseloads visit schedules pain control symptom management wound care IV care and high-tech needs
Educate patients and caregivers on medications treatment options discharge instructions and reportable conditions
Document care accurately using McKesson and coordinate safe transfers discharges and supervisory visits
Requirements
Active Vermont Registered Nurse License Required Upon Acceptance
Minimum 2 years of Hospice RN Home Health or Case Management experience required
Strong hospice assessment care planning symptom management and patient education skills required
McKesson experience preferred
Must be available for Tuesday Saturday schedule some on-call and holiday coverage
