Responsibilities (RE)
Conduct medication reconciliation and resolve discrepancies with care teams
Educate patients on medication use side effects and adherence strategies
Monitor medication adherence and follow up with at-risk patients
Support care transitions including refills prior authorizations and delivery coordination
Maintain accurate documentation in EHR systems
Collaborate with pharmacists to identify drug interactions and improve care plans
Ensure compliance with HIPAA and pharmacy regulations
Participate in quality improvement initiatives
Requirements
Qualifications
Certification: Current Pharmacy Technician License from a recognized certifying body (e.g. PTCB or NHA).
Experience: Previous experience in a pharmacy setting ideally with a focus on care management or chronic disease management.
Knowledge: Strong understanding of pharmacology medication therapy management (MTM) and patient-centered care practices.
Skills
Technology Proficiency: Familiarity with EHR systems pharmacy software and telehealth tools.
Communication: Excellent verbal and written communication skills for patient and provider interactions.
Attention to Detail: Exceptional accuracy in documenting patient information and managing medication orders.
