Job Description
Job Title: Nurse Practitioner
Supervisor: Medical Director
Summary: Diagnoses and treats clients suffering from the disease of addiction by performing the following duties.
Essential Duties and Responsibilities include the following; other duties may be assigned:
- Elicits and records information about patients medical history
- Conducts a physical and initial assessment for all incoming admissions
- Facilitates weekly treatment team visits with clients
- Facilitates continuing care appointments for aftercare clients as needed
- Orders or executes various tests analyses and diagnostic images to provide information on patients conditions
- Analyzes reports and findings of laboratory screenings
- Prescribes medications to clients as needed
- Advises patients concerning diet hygiene and methods of prevention for disease
- Regularly reviews OARRS for client misuse of controlled substances
- Maintains accurate client charts through the EMR
- Reports births deaths and outbreaks of any contagious disease to governmental authorities
- Communicates effectively with all counseling clinical clerical and billing staff when questions or concerns are presented
- Assists in establishing and maintain proper protocols and clinical procedures
- Coordinates with and refers patients to other facilities and physicians for continuing treatment purposes
- Assists to Facilitate family lectures on Tuesdays and Saturdays and coordinates with Medical Director in scheduling lectures

Qualifications: To perform this job successfully an individual must be able to perform each essential duty satisfactorily. Education and/or Experience: Certified Nurse Practitioner degree; Buprenorphine waiver.
Language Skills: Ability to read and interpret documents such as safety rules operating and maintenance instruction and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organizations.
Mathematical Skills: Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals. Ability to compute rate ratio and percent and to draw and interpret bar graphs
Reasoning: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written oral diagram or schedule form.
Computer Skills: To perform this job successfully and individual should have knowledge of database software internet software and word processing software.
Certificates Licenses Registrations: Current active and unrestricted license to practice nursing in the State of Ohio with the Ohio Board of Nursing; malpractice insurance coverage for the facility with occurrence coverage limits of $per occurrence and $aggregate.
An active DEA license with X-waiver.
