Job Summary

The Medication Coordinator (LPN) is responsible for coordinating medication-related services and supporting nursing operations within Minnesota Adult & Teen Challenge (MnTC) programs. Working under the direction of a Registered Nurse (RN) Supervisor Nursing Services or other designated clinical leader this position serves as a medication-focused nursing resource assisting with medication management processes medication reconciliation client education documentation provider communication and continuity of care activities.

The Medication Coordinator (LPN) serves as a resource for medication-related matters and helps ensure compliance with applicable nursing standards Minnesota Department of Human Services (DHS) requirements Minnesota Department of Health (MDH) regulations and MnTC policies. This position supports individuals receiving substance use disorder and behavioral health services while promoting client safety wellness and treatment outcomes.

Essential Job Responsibilities:

  • Review and reconcile medications upon admission transfer discharge and return from outside medical appointments.
  • Coordinate medication ordering refills prior authorizations and prescription renewals in collaboration with pharmacies medical providers and nursing staff.
  • Monitor medication inventories and assist with maintaining adequate medication supplies.
  • Review medication orders for completeness and accuracy and communicate discrepancies to the RN prescribing provider or appropriate clinical staff.
  • Assist with implementation and supervising of medication administration and self-administration procedures in accordance with organizational policies and applicable regulations.
  • Educate clients regarding medications potential side effects medication schedules and adherence to prescribed treatment plans within the scope of practice of an LPN.
  • Communicate with physicians pharmacies hospitals clinics and other healthcare providers regarding medication-related concerns and continuity of care needs.
  • Review medical records appointment summaries discharge instructions and provider recommendations to ensure timely implementation of treatment orders.
  • Observe document and communicate medication-related concerns client responses and changes in condition to the RN prescribing provider or appropriate clinical staff for further evaluation.
  • Assist with medication audits medication storage reviews inventory counts and quality assurance activities.
  • Identify document and report medication discrepancies medication errors adverse reactions and client concerns according to organizational policy.
  • Monitor and document client health information and communicate significant findings to the RN or appropriate medical provider.
  • Participate in interdisciplinary treatment team meetings and communicate relevant medication and healthcare information to treatment staff.
  • Assist with training staff on medication-related policies procedures medication self-administration protocols and healthcare-related practices.
  • Maintain accurate and timely documentation in electronic health record systems and other required platforms.
  • Support compliance with applicable nursing practice standards DHS licensing requirements MDH regulations and organizational policies.
  • Perform other duties as assigned.

Required Skills & Experience (Minimum Qualifications):

  • Current Licensed Practical Nurse (LPN) license unrestricted and in good standing in the State of Minnesota. Must maintain licensure in accordance with the Minnesota Nurse Practice Act and Minnesota Board of Nursing requirements.
  • Graduate of an accredited practical nursing program.
  • One year of nursing experience preferred; experience in behavioral health substance use disorder treatment residential care correctional healthcare or community health settings preferred.
  • Knowledge of medication administration principles medication reconciliation processes nursing documentation requirements and applicable nursing standards and regulations.
  • Ability to work collaboratively under the direction of a Registered Nurse while exercising sound judgement within the scope of LPN license.
  • Strong organizational time management and attention-to-detail skills.
  • Ability to effectively communicate with clients families healthcare providers pharmacies and interdisciplinary team members.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Comfort and compassion in working with individuals experiencing mental health conditions substance use disorders and other behavioral health challenges.
  • Demonstrate MnTCs mission Culture of Honor core values and Putting Honor in Action principles through daily interactions fostering a welcoming respectful and relationship-centered environment.
  • At least one year of freedom from problematic substances.
  • Current CPR and First Aid certification or ability to obtain certification within three months of hire.
  • Must successfully complete all required training and demonstrate competency in client rights First Aid CPR and other training requirements applicable to the position and program licensure.
  • Must pass a DHS background study as required under MN Statutes Chapter 245C and 245G.

  • Prolonged periods of sitting standing walking bending reaching and working on a computer.
  • Ability to occasionally lift and carry up to 30 pounds.
  • Ability to safely assist clients with mobility needs activities of daily living and emergency situations.
  • Ability to respond promptly to emergencies and move throughout the facility as needed to ensure client safety.

Regular on-site attendance required to support client care interdisciplinary collaboration and operational needs

Target Compensation (Negotiable based on Qualifications):

$62000-$65000/year
