About the Role
We are seeking an experienced Licensed Practical Nurse / Licensed Vocational Nurse for a 13-week travel opportunity in Watertown NY. This role will support the sub-acute rehab floor in a long-term care setting caring for patients with dementia chronic diseases diabetes and other ongoing medical needs.
Responsibilities
- Provide direct nursing care to residents and rehab patients
- Administer medications and treatments as ordered
- Monitor patient condition and report changes to the care team
- Document care accurately using PointClickCare eMAR
- Support residents with chronic disease management and daily care needs
- Maintain patient safety infection control and quality care standards
Requirements
- Active New York LPN/LVN license required
- BLS certification from the American Heart Association required
- 1 3 years of nursing experience required
- SNF experience accepted; LTC/sub-acute rehab experience preferred
- Experience with PointClickCare eMAR preferred
- Must meet 100-mile travel radius requirement
