About the Role
We are seeking an experienced Licensed Practical Nurse / Licensed Vocational Nurse for a 13-week travel opportunity in Watertown NY. This role will support the sub-acute rehab floor in a long-term care setting caring for patients with dementia chronic diseases diabetes and other ongoing medical needs.

Responsibilities

Provide direct nursing care to residents and rehab patients
Administer medications and treatments as ordered
Monitor patient condition and report changes to the care team
Document care accurately using PointClickCare eMAR
Support residents with chronic disease management and daily care needs
Maintain patient safety infection control and quality care standards

Requirements

Active New York LPN/LVN license required
BLS certification from the American Heart Association required
1 3 years of nursing experience required
SNF experience accepted; LTC/sub-acute rehab experience preferred
Experience with PointClickCare eMAR preferred
Must meet 100-mile travel radius requirement
