Schedule: Monday Friday 8:00 AM 5:00 PM
Contract: 13 weeks

About the Role

We are hiring an Occupational Therapist for a 13-week contract in Grand Junction CO. This role supports outpatient and home health services helping elderly and frail patients improve daily living skills safety independence and quality of life.

Responsibilities

Evaluate patients and create individualized occupational therapy treatment plans
Provide therapy focused on daily living skills mobility safety and independence
Support elderly and frail patient populations in a PACE-related care model
Provide services in outpatient and home health settings
Educate patients and caregivers on adaptive strategies and safe care techniques
Collaborate with interdisciplinary care teams to support patient goals
Monitor progress and adjust treatment plans as needed
Complete accurate documentation and follow care standards

Requirements

Bachelors Masters or Doctorate in Occupational Therapy required
Active Colorado Occupational Therapist license required
NBCOT registration required
Minimum 2 years of Occupational Therapist experience required
At least 1 year of experience with elderly and frail populations required
Outpatient or home health experience preferred
Valid drivers license reliable transportation and auto insurance required
Ability to work Monday Friday day shift
