Day Shift: Monday to Friday 8am-4:30pm

Together we are committed to breaking the cycle ofhomelessness and helping our clients achieve long-term Summary: The Case Manager for the Encampment Resolution Funding (ERF) program provides outreachservices to unsheltered individuals and households residing in San Mateo Countys Critical Homeless EncampmentZones (CHEZ). The primary focus is on CHEZ sites in San Mateo Burlingame San Bruno South San Franciscoand Menlo Park with flexibility to serve other ERF clients as needed. Key responsibilities include outreach andengagement case management participation in Multi-Disciplinary Team (MDT) meetings and rapid response tocommunity inquiries or referrals. The goal is to assist individuals in accessing housing and connecting to criticalresources such as shelter health services and other support systems.
Case Management Duties:
- Provide intensive field-based case management to unsheltered individuals maintaining a caseload of 1520 clients with a minimum of twice-monthly in-person contact.
- Develop and implement client-centered housing plans to assist individuals in rapidly transitioning fromhomelessness to permanent housing following Housing First principles.
- Conduct rapid response outreach and engagement within eight (8) business hours of the request.
- Support collaboration through active participation in multidisciplinary team (MDT) meetings with keystakeholders including law enforcement San Mateo County BHRS Street Medicine homeless sheltersand other safety net providers while ensuring compliance with confidentiality policies and proper consentprotocols for information sharing.
- Engage clients in trauma-informed harm-reduction services supporting access to shelters health servicesand housing resources without prerequisites such as sobriety or income.
- Assist clients in obtaining documents completing housing applications and navigating referrals forservices like substance use treatment mental health care and permanent supportive housing.
- Collaborate with multidisciplinary teams service providers and housing programs to ensure warmhandoffs and continuity of care when clients transition between programs.
- Support clients post-housing transition with ongoing case management and resource connections for up to ayear to promote stability and goal achievement.
- Monitor and reassess client progress coordination with community partners and maintaining consistenthigh-quality and up-to-date documentation and data.
- Other duties as assigned by Supervisor.
Qualifications
- Bachelors degree in social work psychology or related field with a minimum of two years casemanagement experience. Relevant lived experience can be substituted for education.
- Excellent communication and interpersonal skills; proven ability to collaborate with community partners
- Understanding of the issues faced by homeless individuals and other marginalized populations in crisis
- Availability to work occasional evenings and weekends.
- Experience working with mentally ill and dually diagnosed clients highly desirable
- An understanding of ideas and practices that promote diversity equity and inclusion; and a commitment toongoing professional development in this area.
- Bi-lingual Spanish language capabilities a plus
