Across our 60 locations we provide programs in a variety of service areas including workforce development children youth families seniors and homeless prevention.
Our family shelters are safe welcoming spaces where families can stay temporarily while they work toward long-term stability. Program staff provide compassionate support life-enhancing services and community-based resources to help empower each family. Together we guide families on their journey back to independence and into permanent housing.
RESPONSIBILITIES:
Supervise a team of two Case Managers through weekly individual and monthly group meetings.
Review physical and virtual case records and reports at least monthly.
Direct staff interventions in difficult or crisis situations.
Maintain and monitor program reports to ensure statistical goals are met.
Monitor program for quality assurance and contract compliance.
Provide each Case Manager with yearly performance appraisals.
Assist in the development and implementation of services for clients.
If/when short staffed maintain a case load and provide direct services to clients including intake engagement assessment service plan development referral advocacy counseling exit summaries and follow up.
Interview potential new staff and put together hiring paperwork.
Train new staff inall ofthedifferent aspectsof case management including using CARES conducting unit inspectionsandsubmittingthem intoSalesForce creating andmaintainingclient records etc.
Represent the agency at meetings and events.
Act as a liaison betweenBronxWorksand other CBOs the area community district and government agencies.
Perform additional duties as assigned by the Director of Social Services and/or Residence Director.
Evaluation Categories:
Monthly Binder/CARES review completion as well as regular follow up
Quality and quantity of supervision ensuring CM staff submit their monthly reports on time.
Ensure that ILP completion rates are high by tracking and monitoring.
Ensuring that ILP contents area appropriate and staff are not copying and pasting their progress notes.
Ensuring that case notes and ILPs are done within a timely manner.
QUALIFICATIONS:
A masters degree in social work is required.
New York State LMSW certification or ability to obtain licensure within 6 months of hire.
Prior Supervisory experience working in a social service agency preferred.
Strong oral and written communication time management and organizational skills are necessary.
Proficiency in Microsoft Office suite and other standard business technology required.
PHYSICAL REQUIREMENTS:
Ability to use a computer for prolonged periods
Ability to occasionally lift and/or move up to 10 pounds
Ability to stand walk or sit for long periods of time
Ability to climb five flights of stairs if required conduct home visits and/or fieldwork
Ability to bend and retrieve objects and/or documents
Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work
