Location: Napa CA
Pay Rate: $56.00 $58.00/hr
Schedule: Day shift
Contract: 13 weeks
Guaranteed Hours: 36 hours/week
Experience: Minimum 1 year
About the Role
We are hiring a Physical Therapist for a 13-week contract in Napa CA. This is a home health role covering assigned territories including Vacaville Fairfield Rio Vista Suisun City Dixon Vallejo Benicia Napa American Canyon and Yountville.
Responsibilities
- Provide physical therapy services in a home health setting
- Complete evaluations routine visits reassessments recertifications and discharges
- Develop and follow individualized patient treatment plans
- Support mobility strength balance safety and functional independence
- Travel within assigned territory and outside the area as needed
- Complete patient documentation accurately and timely
- Communicate with patients physicians and care team members
- Meet weekly productivity and visit expectations
Requirements
- Active California Physical Therapist license required
- Minimum 1 year of Physical Therapy experience required
- Home health experience preferred
- BLS/BCLS certification required
- Valid drivers license auto insurance and auto registration required
- Must be willing to travel within assigned territory
- Weekend coverage may be required
- Must be willing to see COVID and COVID PUI patients
- Required health documents include TB MMR Varicella HepB TDap flu drug screen and physical
