Malpractice and disciplinary background issues: If your provider has more than 500K in malpractice or more than 2 cases please include first and third party documentation on the cases. Credentialing/Privileging: Privileging generally takes 60-90 days. It s imperative that the provider is available to send their DOB SSN License contact information and a signed form sent during the offer process within 72 hours of signing.
