Are you a Senior Support Worker Team Leader or experienced Support Worker looking for a local permanent role where you will help coordinate specialist learning disability support for both adults and children across North Somerset
This is an excellent opportunity to join our Learning Disability team where you will support the coordination of adult supported living services alongside community outreach and respite care for children and their families.
Within our adult services you will help coordinate teams of Support Workers delivering person-centred care within supported living homes. You will assist with staff rotas supervisions spot checks medication observations and the preparation of care plans and risk assessments.
You will also help coordinate childrens outreach packages across North Somerset. This support is mainly respite care designed to give families a helping hand while enabling children with learning disabilities to enjoy activities at home and in the community.
Childrens outreach shifts can range anywhere from 3 to 8 hours and may take place between 7am and 8pm although most support is delivered between 9am and 6pm.
We have excellent opportunities for training and development including the opportunity to start or complete a Level 3-5 NVQ in Health and Social Care.
Network Healthcare is a nationwide care provider specialising in domiciliary care live-in care complex care supported living and specialist learning disability support.
This role would suit someone with a strong background in health and social care who is looking to progress into a varied coordination role supporting both adults and children.
THE ROLE:
Coordinating adult supported living services and childrens outreach packages across North Somerset
Planning staff rotas and responding to sickness changes and emergency cover
Preparing and updating care plans risk assessments and other service documentation
Supporting and supervising staff with shared on-call responsibility every other week
THE PERSON:
Experience working in health and social care is essential
Experience supporting adults or children with learning disabilities would be beneficial
Strong organisational skills and confident coordinating staff and support packages
