Requirement description :
- Home Health experience REQUIRED
- Valid drivers license and car insurance REQUIRED
- Copy of agencys IL nursing license included in submission REQUIRED
- Will consider someone with strong Med Surg experience with some Home Health experience
- COVID and FLU vaccination required DECLINATIONS WILL NOT BE APPROVED **Please add vaccination status in the profile**
- OASIS and start of care experience strongly preferred
- Manager is wanting to review local candidates! If the candidate is too far from the territory they will redirect.

- 2 Supervisory references
- Certifications (must be active at time of submission)
